Health fund membership card
X-rays, scans and previous operation reports
Medication list
A completed copy of your Patient Registration Form
Alternatively, please arrive 15 minutes early to complete your paperwork

Clinic access is via the Ground floor on South St or from the Link Bridge on Level 1 at St George Private Hospital

If you’re driving, entry to the carpark is via South Street, under St George Private Hospital.

There are several parking lots available on South Street, Hogben Street, and Derby Street. Parking is also available on unmetered spots on the street for 2 hours.

Kogarah Train Station is an approximately 7 minute walk away via Montgomery Street.
